I was reading The Wish List on the train this morning and had to stop because I couldn't read on until I'd given you my guess. They're outside Meg's old house and just about to punch possessed Franco, which is the third wish that Lowrie decided to let Meg have.

Tom's prediction:

They have a big fight that tires Lowrie out so much that he starts to die early or something, or Meg uses her energy and has to leave earth. She's worried about Lowrie dying and not having completed the list for him and he'll say:

She'll be like, what? There's the fourth wish, we didn't do it.
